Seems that once accepted the mentoring organization needs to fill this form:

@genete: We could certanly use the SFC now haha. No idea how that works either but if we get accepted I can ask my friend for some guidance.

IIRC the task to provide documents is delegated to Administrator. Unfortunately I can't find Administrator Agreement link on GSoC site now to put exact quote.

In 2012 they used the pretty same agreement.
